We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, or most major credit cards.


If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.

Dr. Hazlett is currently pursing a post-doctoral certification in Functional Neurology. He has completed over 2 years and 235 credits of post-doctoral classes with a focus on functional rehabilitation of neurological based disease and dysfunction of soft lesions including but not limited to mTBI, vestibular dysfunction and deficit based functionality of neuromuscular systems. 

Having his background based in exercise rehabilitation, he is able to recognize and utilize a multitude of different rehabilitation techniques, neurological rehabilitation techniques and physiotherapy modalities in conjuction with specific chiropractic manipulations to customize care. This approach provides his patients with a robust and quality care plan to ensure an expedited and complete recovery.












Donald E. Deibler, D.C.

Dr. Deibler is a native of Lancaster County. He did his undergraduate work at Millersville and Palmer Colleges. His Chiropractic degree is from Palmer College of Chiropractic. Dr. Deibler has been active in state and local organizations, and is a member of the Lancaster County and Pennsylvania Chiropractic Associations. He has done further post-graduate work in Adjuntive Physiotherapy, Acupuncture, and Spinal Stress Analysis. In active practice for 40 years, his partice includes auto and work related injures, as well as general practice.

Cassidy Baer

Cassidy is the Practice Coordinator. She is responsible for processing your medical claims to both primary and secondary payers. Her goal is to proivide accurate information to both you the payer and the patient. She reviews patient's bills for accuacy and completeness and obtains any missing information. If you have any questions about your insurance, it is best to contact your insurance company directly. However, if you have any further questions you can contact Cassidy at 






Kayla Ulrich

Kayla Ulrich is a Licensed Massage Therapist, LMT. She graduated from Lancaster School of Massage. She is a member of the ABMP (Associated Bodywork & Massage Professionals)and has been practicing massage therapy for 16 years. Her integrative style combines flowing Swedish Massage, Deep Tissue, Myofacial Release, and Trigger point therapy.

Kayla works to assist her clients in relieving stress related tension, reducing chronic pain, reduicing recovery time of injuries , and restoring mobility. She believes in the importance of tailoring each session to the clients individual needs.

Melissa Rhoads

Melissa has worked for Chiropractic Associates since January 2015. She enjoys coming to work and being a part of the healing process of our patients. She is a mother of three boys which keeps her busy outside of her job here. She is a huge health advocate and enjoys seeing people take steps to better health. She spends a lot of her time reading and researching. Some of her life interests include; crocheting, food, movies, some sports and anything outdoors. 






Wanda Torres

Wanda joined our team in September 2015. She graduated with her nursing degree in 2010. She is bilingual and has been a great asset to the team so far, by translating between doctors and patients for accurate care. In the future, she will be joining our doctors in seminars, to encourage those who are in need of Chiropractic care and have the inability to seek it appropriately.
